About The Toolkit

This CSTE toolkit is built upon a comprehensive, multi-year strategic plan developed by the Council of State and Territorial Epidemiologists (CSTE) in collaboration with CDC, national public health associations, and STLT epidemiologists. Anchored in CDC’s broader Data Modernization Initiative and Public Health Data Strategy, it responds to the unique challenges posed by chronic disease surveillance and provided strategies and actions to make meaningful progress towards a modernized future.

Aligned with the strategic plan’s core strategies, the toolkit facilitates workforce development and capacity-building and provides resources to support streamlined data flows, interoperable data systems, integrated governance processes, and enhanced policies.

Users will find implementation guides, templates, success stories, training resources, role-specific talking points, and more! The embedded resources aim to support actionable change and foster peer learning and connection.
